Manage Cohort

Cohort list page used by instructors to manage group-based learning cohorts across all of their courses. A course search field at the top filters the table to a selected course. The top has a Create Cohort button (scoped to the selected course); each row has Edit and Delete buttons.

1. When the user changes filter_course_id the table reloads automatically; if no course is selected, cohorts from all courses are shown and the "Course" column always shows the course title per row.
2. Self-pace mode — Create / Edit / Delete buttons are all disabled (not hidden), and Require Approval is shown read-only (defaults off; only a Super Admin may change it) + note "Managed automatically by the system".
3. After redirect back from a successful save (SCR-LMS.17) or delete (SCR-LMS.18), the system shows a success toast that auto-dismisses after 3 seconds.
